Job summary

PruittHealth Corporation is recruiting a Program Coordinator for Community Services to drive CCSP training and implementation across partner agencies. The role focuses on developing guides, workflows, and ensuring compliance with state requirements while coordinating with agencies and internal teams.

The candidate will manage reporting, trackers, policy updates, and monthly meetings to keep CCSP initiatives aligned with contractual and regulatory standards.

Responsibilities

• Coordinate training and implementation of CCSP at PruittHealth Home First, Inc. Case Management agencies
• Develop training courses, training guides, and process workflows for CCSP implementation
• Monitor referrals and coordinate communication with Area Agency on Aging (AAA)
• Manage and strategize program development for CCSP to grow and meet operational goals
• Demonstrate knowledge of contractual, legal, and regulatory requirements with Georgia’s Medicaid Elderly and Disabled Waiver Program (EDWP)
• Provide assistance and guidance to the PruittHealth Home First offices to ensure CCSP sites deliver services in accordance with contractual, legal, and regulation requirements
• Submit Programmatic Reports to the Department of Community Health (DCH)
• Generate and review Harmony Advance Reporting (HAR) reports monthly
• Maintain trackers monthly (Supervisory, Scheduler, Eligibility Checks)
• Demonstrate knowledge of all DCH Policy Manuals
• Maintain and update PolicyTech documents and forms for CCSP
• Attend meetings (regional, Care Division, Provider Network Meetings, Train the Trainer)
• Present at Corporate Conferences regarding any CCSP updates
• Host monthly CCSP calls with all CCSP staff
• Assist with questions (clients, caregivers, providers, staff)
• Notify offices of No Bill and Billable Late Lock-In
• Follow up with Medicaid status
• Follow up on Prior Authorization (PA) billing issues
• Set-up MOUs with AAAs to receive MFP payment
• Follow up with Referrals
• Resolve PA issues
• Manage user access in Microsoft Teams – Home First
• Manage user accesses in Harmony – Pruitt CCSP sites
• Upload pertinent documents from DCH’s Train the Trainer to Microsoft Teams – Home First
• Coordinate between ADRC and Pruitt Contracting team re: annual MOU contracts for MFP assessments
