Job summary

PruittHealth is seeking a Program Coordinator for Community Services to lead CCSP implementation across Home First partners. You will coordinate training, develop guides, monitor referrals, and ensure compliance with Medicaid EDWP requirements in Georgia.

Daily duties include submitting reports to the Department of Community Health, preparing HAR and cost reports, maintaining trackers, and coordinating with ADRC, AAAs, and provider networks.

Responsibilities

• Submit Programmatic Reports to the Department of Community Health (DCH).
• Generate and review HAR reports monthly per SOPs.
• Maintain monthly trackers for Supervisory, Scheduler, and Eligibility Checks.
• Stay current with DCH Policy Manuals for CCSP.
• Update PolicyTech documents and CCSP forms.
• Attend regional and provider network meetings; train the trainer.
• Present CCSP updates at corporate conferences.
• Host monthly CCSP calls with staff.
• Assist clients, caregivers, providers with questions.
• Notify offices of No Bill and Billable Late Lock-In.
• Follow up on Medicaid status and PA billing issues.
• Set up MOUs with AAAs for MFP payments.
• Follow up on referrals; resolve PA issues.
• Manage user access in Microsoft Teams and Harmony.
• Upload DCH Train the Trainer docs to Teams.
• Coordinate annual MOU contracts for MFP assessments.
