Job summary

Benchmark Human Services in Swedesboro, NJ, is seeking a Program Coordinator to supervise and train staff, participate in ISP/PCP development with an interdisciplinary team, and ensure high-quality direct care and timely documentation. You will coordinate repairs, liaise with stakeholders, and support clients across home and community settings.

The role requires at least 5 years working with individuals with developmental disabilities in group homes, 7+ years of supervisory experience, a valid

Qualifications

  • Minimum 5 years’ experience working with individuals with developmental disabilities in a group home setting.
  • 7+ years of supervisory experience.
  • Valid driver’s license and auto insurance.

Responsibilities

  • Supervise, train, and provide guidance to staff in line with state/federal regulations and Benchmark policies.
  • Participate in interdisciplinary team development of each client’s ISP and PCP.
  • Provide direct care to clients as needed.
  • Complete daily/weekly/monthly/yearly documentation.
  • Coordinate repairs and maintenance of consumers’ homes.
  • Communicate effectively with stakeholders.
  • Assist in hiring and management of direct care staff.
  • Schedule and support clients’ medical appointments and procedures.

Responsibilities:
  • Supervise, train, and provide guidance to the staff in accordance with state and federal regulations, Benchmark Human Services policies, the Supported Living policy, and procedure manual.
  • Participate as part of an interdisciplinary team in the development of each client's ISP and PCP.
  • Participate in direct care for each client.
  • Complete daily, weekly, monthly, and yearly documentation.
  • Facilitate all repairs and required maintenance of the consumer's homes.
  • Communicate effectively with stakeholders.

Requirements:
  • High School Diploma or Equivalency required, Degree preferred.
  • 5 years' Experience working with individuals with developmental disabilities in a group home setting.
  • 7+ years Supervisory Experience
  • Valid driver's license and auto insurance.
