Job summary

Benchmark Human Services is seeking a Program Coordinator in Swedesboro to supervise staff, participate in interdisciplinary care planning, and coordinate client ISP/PCP development.

You will handle direct care activities, maintain comprehensive documentation, and manage scheduling for medical appointments, while ensuring compliance with policies, regulations, and quality standards. This role offers growth and a supportive team environment.

Responsibilities

• Supervise staff in alignment with state/federal regulations and policies.
• Collaborate on ISPs and PCPs within an interdisciplinary team.
• Provide direct care to clients as needed.
• Maintain accurate daily, weekly, monthly, and yearly documentation.
• Oversee repairs and maintenance of client homes.
• Communicate effectively with stakeholders.
• Assist in hiring and supervising direct care staff.
• Schedule medical appointments and procedures for clients.

Responsibilities:

• Supervise, train, and provide guidance to the staff in accordance with state and federal regulations, Benchmark Human Services policies, the Supported Living policy, and procedure manual.
• Participate as part of an interdisciplinary team in the development of each client's ISP and PCP.
• Participate in direct care for each client.
• Complete daily, weekly, monthly, and yearly documentation.
• Facilitate all repairs and required maintenance of the consumer's homes.
• Communicate effectively with stakeholders.
• Participate in hiring and management of direct care staff.
• Schedule and assist clients in all medical appointments and procedures.

Requirements:

• High School Diploma or Equivalency required, Degree preferred.
• 5 years' Experience working with individuals with developmental disabilities in a group home setting.
• 7+ years Supervisory Experience
• Valid driver's license and auto insurance.
