Job summary

Northeast Arc is seeking a dedicated professional to supervise and support participants in community-based settings, including day and work support programs. The role emphasizes active movement, accompanying individuals to recreational activities and volunteer opportunities, with frequent walking and standing throughout the day.

You will oversee caseloads, set individualized goals, and document progress in a timely manner.

Qualifications

  • High School Diploma or equivalent required. 1 year of experience working with intellectually and/or developmentally disabled adults or related experiences preferred.
  • Excellent organizational and interpersonal skills. Effective communicator in person and in writing. Ability to communicate in English and/or ASL.
  • Valid driver’s license and reliable transportation is required.
  • Must be able to lift up to 50 lbs. as needed.
  • Must be able to stand/walk up to one hour uninterrupted.

Responsibilities

  • Evaluate individuals progress periodically and make adjustments as needed to improve outcomes.
  • Evaluate and determine program participants’ interests and develop individual skills.
  • Provides assigned caseload oversight by attending scheduled meetings, maintaining regular and professional contact with the participant’s team, completing all required documentation, and ensuring the participant’s progress in the program as determined by his or her goals.
  • Creates individual goals to reflect the participant’s vision regarding skill development.
  • Maintains all required documentation within required timeframes to include: Daily Data, Monthly/Quarterly Progress Notes/6-month summaries in HCSIS, Assessments, Confidential Record updates.
  • Provides transportation to participants served in a personal or agency vehicle when one is available.
  • Assists in obtaining appropriate volunteer placements for participants served and provides ongoing coaching support in these placements to ensure continued success.
  • Attends and completes all required training assigned by the supervisor.

Job Summary:

The employee is expected to spend the majority of their workday actively supporting individuals in community settings, community-based day and work support program, including accompanying individuals to recreational activities, volunteer opportunities, and other community-based events. The role requires frequent walking, standing, and movement throughout the day.
