Job summary

City of Hope is seeking a professional coder to review medical records and assign ICD-10-CM diagnoses and CPT-4 procedure codes for professional fee services. You will ensure accurate sequencing and adherence to coding guidelines, while consulting physicians to resolve ambiguities and maximize appropriate reimbursement.

Responsibilities include using encoder tools, maintaining up-to-date knowledge of ICD-10-CM and CPT-4, and producing productivity and quality reports.

Qualifications

• Two years of coding experience of professional fees (physician/medical office).
• Thorough knowledge of medical terminology/anatomy/ physiology.
• Comprehensive understanding of professional fee coding principles and modifiers.
• Current knowledge, training and experience in ICD-10.

Responsibilities

• Reads and interprets medical record documentation to identify diagnoses, conditions and procedures for coding.
• Clarifies conflicting or ambiguous information by consulting the physician.
• Applies ICD-10-CM guidelines to select diagnoses and procedures.
• Uses CPT-4 guidelines to locate correct service codes and sequence them.
• Assigns diagnoses and procedures and sequences them for professional fees.
• Applies anatomy knowledge to assign accurate codes for diagnoses and procedures.
• Follows Basic Coding Guidelines for professional fee physician coding.
• Reviews CPT-4 guidelines and notes to locate correct codes for services.
• Adheres to payer guidelines to ensure optimal reimbursement.
• Keeps encoder/grouper tools up to date and used as reference.
