Job summary

Habitat Health is seeking a Product Manager, Strategic Technology Applications to own the product strategy, roadmap, and cross‑functional delivery for the technology portfolio powering our growing PACE organization. This remote role requires periodic travel for on-site go‑lives and support, reporting directly to the VP of Product.

You will own end‑to‑end lifecycle across call/communications, CRM (Salesforce), and EHR (Epic), partnering with analysts and vendors, maintaining the backlog in Jira,

Responsibilities

Own product vision, strategy, and roadmap for a portfolio of strategic technology applications including call/communications, CRM, and EHR.
Translate stakeholder needs into a prioritized backlog and roadmap, balancing quick wins with long-term investments.
Define and track success metrics—adoption, utilization, satisfaction, and impact—to guide iteration.
Maintain portfolio health, dependencies, and integrations to ensure data integrity across platforms.
Run discovery with clinical, operational, and growth stakeholders to surface pain points and shape requirements.
Partner with analysts and external vendors to scope, prioritize, and deliver changes and new features.
Own backlog grooming and sprint planning using Jira; lead implementations for new center launches.
Build repeatable playbooks to support Habitat Health's continued growth.
