Job summary

VitalCaring Group is seeking a PRN Weekend Home Health RN to lead Starts of Care and ensure smooth weekend transitions for patients in their homes. This field-based role rewards autonomy and clinical judgment as you assess, plan, and implement care in real-time.

You’ll work with a supportive interdisciplinary team, manage a caseload, document thoroughly in the EMR, and help patients achieve better outcomes while balancing work-life needs with flexible weekend scheduling.

Qualifications

• Active RN license and CPR certification required.
• Graduate of an accredited nursing program.
• Minimum one year RN experience, home health or acute care preferred.
• Strong clinical judgment and independent decision-making.
• Excellent communication and patient-centered approach.

Responsibilities

• Conduct in-home assessments to evaluate clinical status and goals of care.
• Develop and manage individualized care plans with physicians and teams.
• Deliver skilled nursing care, including medications and treatments.
• Coordinate across therapists, aides, and physicians as case manager.
• Monitor progress and adjust care based on changing conditions.
• Educate patients and families on disease management and independence.
• Coordinate transitions and discharge planning to reduce readmissions.
• Maintain timely documentation in the EMR.
• Supervise Home Health Aides and support team.
• Participate in on-call rotation as appropriate.
