Job summary

VitalCaring Group in the United States welcomes a PRN Weekend Home Health Registered Nurse to lead care in patients' homes. You will manage plans of care, coordinate with physicians and therapists, and drive outcomes from admission to discharge, delivering skilled nursing and education to support recovery and independence.

Qualifications

• Active RN license (or compact eligibility) and CPR certification.
• Graduate of an accredited nursing program.
• Minimum one year RN experience (Home Health, Med-Surg, ICU or Acute Care preferred).
• Strong clinical judgment and ability to think independently.
• Excellent communication skills and patient-centered approach.
• Proficiency with EMR systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care.
• Develop and manage individualized care plans in collaboration with physicians and care teams.
• Deliver skilled nursing care, including medication management, treatments, and interventions.
• Serve as the case manager, coordinating care across therapists, aides, and physicians.
• Monitor patient progress and make clinical decisions based on changing conditions.
• Educate patients and families to support disease management and independence.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely, accurate documentation in the EMR.
• Supervise and support Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
