Qualifications

• Graduate degree as a Nurse Practitioner or Physician Assistant.
• Current Wisconsin RN license for NP or PA license.
• CPR/BLS certification required.
• 1–3 years clinical experience preferred.

Responsibilities

• Provides assessment, evaluation, and management to patients with minor, acute, chronic, and episodic illness or injuries.
• Collaborates with patients to review health background and determine needs to advise on health plans.
• Provides case management and referrals to needed services.
• Conducts follow-up calls and provides health counseling and education.
• Orders diagnostic tests, prescribes pharmacological agents, and documents in EMR.
• Maintains and reviews patient records and coordinates patient care plans.
