Overview

Position: Speech Pathologist/Audiologist for Keller, Southlake, Alliance area. This role provides speech pathology services on an intermittent basis to clients in their homes in accordance with physician's orders.

Responsibilities
  • Evaluates the client\'s level of functioning and hearing; recommends mechanisms to enhance hearing ability.
  • Provides initial and ongoing comprehensive assessments of the client’s needs, including Outcome and Assessment Information Set (OASIS) assessments.
  • Establishes a Speech Therapy treatment plan in consultation with the physician and revises it with physician approval as necessary.
  • Assists in development and implementation of the interdisciplinary client care plan as it pertains to Speech Therapy.
  • Ensures physician orders are appropriate and discusses necessary changes.
  • Communicates with other team members and, when appropriate, instructs them in speech pathology techniques for use with the client.
  • Teaches, supervises, and counsels the family and client in the total Speech Therapy program and related problems at home.
  • Evaluates the client and home situation to determine required instruction and assistance from the family and other agency/community services.
  • Arranges for outpatient services for procedures that cannot be performed in the home.
  • Records evaluation data, treatments, and client responses in a timely manner.
  • Supervises the Home Health Aide every two weeks when Speech Therapy is the only professional discipline involved in the client\'s care.
  • Records and reports to the physician the client\'s reaction to treatment and any changes in condition.
  • Participates in periodic revision of the physician\'s plan of treatment and keeps the physician informed of the patient\'s status, obtaining additional orders if necessary.
  • Communicates with the physician at least every 30 days or when changes occur regarding the client\'s status and obtains additional orders if necessary.
  • Evaluates the effectiveness of speech pathology intervention and updates the client care plan as needed.
  • Writes clinical/progress notes on the date of the visit and incorporates them into the client\'s record within 72 hours.
  • Demonstrates commitment to self-development through ongoing education and professional involvement.
  • Confirms weekly scheduling of visits with the Director of Nursing to coordinate the patient\'s visit schedule.
  • Participates in agency activities, case conferences and meetings as appropriate.
  • Participates in discharge planning activities and completes a Speech Therapy Discharge Summary.
  • Adheres to established agency policies and procedures.
  • Participates in Quality Assessment and Performance Improvement activities as assigned.
  • Attends all patient care conferences as scheduled.
  • Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, visit notes and significant change in condition within agency-defined timeframes.
Assists in Agency Admission Process
  • Obtains a medical history, particularly relating to the present condition.
  • Evaluates the client and home situation to determine instruction and assistance available from the family and other agency/community services required.
  • Ensures physician orders are appropriate and discusses necessary changes.

  • American Speech-Language-Hearing Association membership preferred.
  • Licensed or registered by the State, if required.
  • At least one year of institutional experience.
  • At least 2 years of home health experience and at least one year of supervisory experience (2 years preferred).
