Job summary

Avera Sacred Heart Pavilion in Yankton, SD is seeking a Speech-Language Pathologist to evaluate patients and develop treatment plans for medically necessary speech therapy services addressing deficits in speech, language, cognition, voice and dysphagia. The role includes direct therapy in home health/hospital settings and collaboration with the interdisciplinary team.

Responsibilities include patient assessment, individualized treatment planning, documentation, and communication with families

Qualifications

• CCC-SLP certification or active supervised fellowship
• Active state license required upon hire
• 1–3 years of speech-language pathology experience in hospital, SNF, outpatient or home health settings

Responsibilities

• Evaluates patients using formal/informal assessments to determine needs in speech pathology.
• Develops and implements individualized treatment programs.
• Plans therapy aligned with patient abilities and care plans; communicates progress to team.
• Documents evaluation results, treatment plans, progress notes and reports.
• Collaborates with family, physician and interdisciplinary team to coordinate care.

Education

CCC-SLP – ASHA or active supervised fellowship year

Speech-Language Pathologist – Active state license
