Job summary

Enhance Rehabilitation seeks a qualified Speech Language Pathologist to join our growing team. In this role you will evaluate and treat patients in a SNF setting, following an individualized plan of care developed after evaluation.

You will administer standardized speech/language assessments, conduct swallowing evaluations, and develop treatment plans while monitoring progress and communicating with patients, families, and the treatment team.

Qualifications

• Demonstrate the knowledge and skills necessary to provide patient care appropriate in a SNF setting.
• Strong written, oral and interpersonal skills work well with others and to communicate effectively with patients, families, and other treatment team members when gathering and exchanging patient condition information.
• Experience with administering standardized speech/language assessments, conduct swallowing evaluations, developing treatment plans and monitoring patients' progress.

Responsibilities

• Evaluate and treat patients by following an individualized plan of care after evaluation.
• Provide care in a SNF setting with clear documentation and communication with the care team.
• Monitor patient progress and adjust treatment plans as needed, communicating with patients and families as appropriate.
