Qualifications

• Possess current working knowledge of Functional Pathways policies and procedures, clinical programs with application to therapy disciplines.
• Completion of all required screenings, certifications, and licensure. Screening requirements may vary by location.

Responsibilities

• Conduct screenings, evaluations, re-evaluations, discharge planning, and revisions in the established plan of care as supervised by the physician.
• Collaborate with interdisciplinary team to implement therapy plans.
• Document and communicate findings and progress in patient records.
