Job summary

Methodist Healthcare at Home is seeking a PRN Speech Language Pathologist to deliver in-home therapy, focusing on communication, cognitive, and swallowing therapy with a supportive rehab team.

As an SLP, you will evaluate, plan, document progress, educate patients and families, and collaborate with physicians and care managers to ensure smooth transitions and high-quality home-based care.

Qualifications

• Master's Degree in Speech-Language Pathology
• Driver License (DL) required
• SLP licensure/certification as applicable

Responsibilities

• Evaluating communication, cognitive, and swallowing abilities using structured assessments and clinical observation
• Developing and adjusting individualized treatment plans to improve communication and safe swallowing
• Documenting evaluations, progress notes and discharge summaries accurately and timely
• Educating patients and families on therapy goals, home strategies, and progress expectations
• Collaborating with physicians, nursing teams, rehabilitation staff, and care managers for coordinated care and smooth transitions
