Responsibilities and Duties:

1. Perform initial therapy evaluations of each patient within 72 hours of receipt of referral.
2. Teach referral sources about determining and recommending appropriate speech and hearing services.
3. Communicate regularly with physicians, patients, families, and healthcare providers to modify the plan of care based on patient needs.
4. Participate in case coordination with staff nurses and physicians; document appropriately in medical records.
5. Develop treatment plans based on evaluations.
6. Provide clinical follow-up by reporting unfavorable signs and symptoms.
7. Manage own schedule to ensure no missed visits or late evaluations due to scheduling errors.
8. Instruct other health team personnel and patients.
9. Plan for discharge.

Qualifications and Skills:

• Current Texas Speech Language Pathologist License.
• Minimum 1 year of clinical experience in home health.
• Current Certification in BSL/CPR.
• Valid Texas Driver's License and Automobile Insurance.
• Knowledge of WellSky EMR software is a plus.

Salary: Pay Per Visit including mileage reimbursement.
