Cadia Healthcare is seeking a PRN Social Services Assistant to support residents’ psychosocial, mental and emotional needs in a skilled nursing facility. You will work with residents and families to coordinate services and ensure access to needed resources.

The role emphasizes policy adherence, documentation, and timely MDS reporting to meet regulatory requirements. Licensed Social Worker with a BSW is required, and SNF experience is preferred.

Qualifications

• Licensed Social Worker with a BSW is required.
• Experience in a skilled nursing facility is preferred.

Responsibilities

• Review and implement policies regarding resident care and quality of life.
• Develop and maintain a listing of current community resources for residents and families.
• Document progress in meeting psychosocial needs and complete required forms and reports for Administrator.
• Ensure Social Services MDS reporting is completed properly and timely to meet regulatory requirements.
