Job summary

Cadia Healthcare in Wilmington, DE is seeking a PRN Social Services Assistant to support residents and families with psychosocial, mental and emotional needs. You will collaborate with the Social Services team to ensure access to needed services and resources, contributing to a compassionate, resident-centered environment.

Qualifications include a Licensed Social Worker with a BSW; prior skilled nursing facility experience is preferred.

Responsibilities

• Review and implement policies regarding resident care and quality of life.
• Assist in developing facility social work policies.
• Develop and maintain a listing of current community resources for residents and families.
• Document progress in meeting psychosocial needs and complete required forms/reports.
• Ensure that Social Services MDS reporting requirements are completed properly and in a timely manner to meet regulatory requirements.
