Job summary

Avante at Ormond Beach Skilled Nursing and Rehabilitation Center is seeking a PRN Speech Language Pathologist (SLP) to provide and direct SLP services to residents, including assessment, treatment planning, and documentation. Functions under physicians' orders.

The role focuses on residents with communication and swallowing disorders and requires a Master’s degree in Speech-Language Pathology, state licensure or eligibility, and 0-3 years in long-term care. AHCA background screening is required.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Successful completion of the National Certification Examination for S.L.P.
• Current state licensure or license eligible.
• 0-3 years of long term care experience.

Responsibilities

• Provide and direct Speech Language Pathologist services to residents, including assessment, treatment, program planning and implementation, and related documentation.
• Communicate results of assessments to physicians and the care team.
• Document daily care and progress per established guidelines.
