Job summary

Oasis Therapy Services in Prospect Heights, IL is seeking a Speech-Language Pathologist to provide rehabilitation services in a senior care and skilled nursing setting.

You will evaluate residents, develop measurable treatment plans, treat speech, language, cognitive-communication, voice, and swallowing disorders, and collaborate with nursing, physicians, and the rehab team. Active IL SLP license, CCC-SLP preferred, CPR current, and strong clinical documentation are required.

Qualifications

• Active IL SLP license required.
• Master’s or Doctoral degree in Speech-Language Pathology from an accredited program.
• CCC-SLP preferred.
• Current CPR certification.
• Strong communication, organization, and documentation skills.
• Geriatric, skilled nursing, dysphagia, or rehabilitation experience preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans with measurable goals.
• Assess and treat speech, language, cognitive-communication, voice, and swallowing disorders.
• Provide skilled therapy interventions to improve communication, swallowing, safety, and quality of life.
• Monitor residents’ progress and adjust treatment plans as needed.
• Educate residents, families, and caregivers on communication strategies and swallowing precautions.
• Collaborate with nursing, physicians, rehabilitation professionals, and the interdisciplinary care team.
• Complete discharge planning and provide recommendations for continued care.
• Maintain accurate and timely Net Health documentation.
