Job summary

Responsibilities

• Conduct comprehensive assessments to evaluate a patient’s physiologic, psychological, developmental, sociocultural, spiritual, and lifestyle factors.
• Develop and implement individualized care plans and revise as needed.
• Administer medications and treatments safely following physician orders.
• Educate patients and families about health conditions and self-care strategies.
• Collaborate with medical staff to coordinate patient care and ensure communication.
• Maintain accurate documentation in the medical record.
• Evaluate care plan effectiveness and adjust to improve outcomes.
• Ensure infection control and safety compliance.
• Provide emotional support and therapeutic communication.
• Supervise and delegate care within the healthcare team.
• Engage in professional development and performance improvement.
• Respond to code situations promptly.
