Job summary

A Path of Care Home Health in Yukon, OK is seeking a PRN Registered Nurse (RN) to support our local team caring for patients in their homes. Qualifications

• Graduate of an approved school of professional nursing.
• Active RN license in Oklahoma.
• At least 1 year of nursing experience.
• CPR certification (or ability to obtain within 60 days).
• Valid driver's license, reliable transportation, and proof of insurance.
• Ability to travel long distances and adapt to changing schedules.
• Experience in home health, critical care, or emergency care is preferred.

Responsibilities

• Conduct patient visits to assess nursing needs.
• Develop individualized plans of care with physician direction.
• Perform ongoing assessments and revise plans as needed.
• Provide skilled nursing treatments and educate patients and caregivers on disease processes, self-care, and prevention.
• Initiate preventive and rehabilitative nursing procedures as ordered.
• Prepare clinical and progress notes and coordinate services with other care team members.
• Communicate directly with physicians and staff to ensure continuity of care.
• Re-evaluate patient needs and assist with discharge planning.
• Supervise LPNs, vocational nurses, and home health aides when appropriate.
• Practice infection control and universal precautions.
• Report suspected abuse according to state and federal guidelines.
• Assist with office tasks and new-nurse orientation when needed.
