Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current CPR certification.
• Current driver's license, vehicle insurance and access to a dependable vehicle or public transportation.
• Proven ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Complete Long Term Care insurance assessments and collect claimant demographics, physician information, and current medication list.
• Evaluate claimant's functional level of independence including ADL/IADL tasks.
• Perform brief cognitive screening and document findings in a web-based system in real time.
• Use a point of care web application on a laptop or tablet for documentation.
