Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current CPR certification.
• Current driver's license, vehicle insurance and access to a dependable vehicle or public transportation.

Responsibilities

• Complete Long Term Care insurance assessments and obtain claimant demographics, physician information, and a current medication list.
• Evaluate claimant's functional level of independence (ADL/IADL) and perform a brief cognitive screening.
• Document in real time using a point-of-care web application on a laptop or tablet; communicate findings to care teams.
