Heart’n Soul Hospice in Memphis, TN is seeking a PRN RN Case Manager to provide compassionate end-of-life care. You will perform comprehensive assessments, implement and revise the plan of care, and collaborate with the patient, family, physicians, and the IDG to coordinate care.

Requirements include graduation from an accredited Diploma, Associate, or Baccalaureate School of Nursing, a current State RN license, a valid driver’s license with reliable transportation, and ideally two years of RN

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance.
• Two years of experience as an RN preferred in a healthcare/clinical setting.

Responsabilidades

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs.
• Implement the individualized POC and revise the plan as necessary.
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals.
• Attend IDG meetings and coordinate patient care planning with the team.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Timely submission of all documentation.
• Ensure overall compliance with laws and regulations.

RN Case Manager

What You Must Have:

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State Registered Nurse License
• Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
• Two years of experience as an RN preferred in a healthcare/clinical setting

What You Will Do:

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals
• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures

I was recently accepted to the American Academy of Hospice and Palliative Medicine’s (AAHPM) Next Gen Scholars Program as one of 8 scholars comprising medical residents, fellows, and post-graduate students across the country.I’ll link more information to what the program entails below, but I am yet grateful for the opportunities and exposure I am gaining through Heart’n Soul Hospice that I believe is orchestrating my future endeavors and career path.
