Job summary

The Medical University of South Carolina is seeking a Program Coordinator – Case Management to support RN Case Managers and Social Workers across the care continuum. The role coordinates payer communication, transition plan implementation, delivery of patient documentation, and other duties in support of case management.

Under general supervision, you will work with the interdisciplinary healthcare team to ensure smooth operations and adherence to department processes and policies.

Qualifications

• Bachelor’s degree and one year work experience; or high school diploma (or equivalent) and 2 to 4 years work experience.
• Strong organizational and communication skills; ability to work with an interdisciplinary team.

Responsibilities

• Coordinate payer communication with internal teams.
• Support RN Case Managers and Social Workers across the care continuum.
• Assist with transition plan implementation and patient documentation.
• Perform other duties to support case management departmental functions.
