Requires Indiana Pharmacist license or eligibility, ASHP PGY1/PGY2 residency may be required, and 0-3 years of relevant experience. Travel to facilities and weekend rotations may be needed.

Qualifications

• Pharmacist license in Indiana is required or eligible.
• ASHP-accredited PGY1 or PGY2 residency may be required.
• 0-3 years of relevant experience.
• Doctor of Pharmacy (PharmD) or BS in Pharmacy before 2006.

Responsibilities

• Provide supplemental staffing for weekdays, evenings, weekends, and vacation coverage.
• Ensure safety and effectiveness of the medication use process and dispense medications as prescribed.
• Monitor therapy and collaborate with healthcare teams to optimize patient outcomes.
• Maintain medication security and confidentiality of patient information.
