Responsibilities

The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes as prescribed by physicians, complying with applicable laws, regulations, and agency policies.

• Coordinates the total plan of care with other healthcare professionals and ensures continuity of care by planning and exchanging information with physicians, agency personnel, patients, families, and community resources, all within a Point of Care setting.
• Conducts initial and/or comprehensive nursing evaluations, determines primary focus of care, develops the plan within state-specific guidelines, and submits accurate, complete, and timely documentation in collaboration with physician orders.
• Performs comprehensive assessments at designated times and develops appropriate care plans.
• Ensures patients meet home health eligibility and medical necessity guidelines as defined by payers.

Education and Experience

License Requirements:

• Current RN licensure in the state of practice
• Current CPR certification
• Valid Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
