Job summary

Magnolia Home Health in Austin, TX is seeking a Registered Nurse PRN to provide skilled home health care to patients of all ages. You will coordinate with an interdisciplinary team, develop and implement care plans, and administer medications at the patient’s residence or in facilities.

You will perform thorough assessments, document care, educate families, and participate in on-call rotations. Current RN license and CPR are required; a valid driver’s license is needed.

Qualifications

• Graduate of an accredited school of nursing.
• Current State RN license; CPR certification required.
• Driver’s license and proof of auto liability insurance; no OIG Excluded Provider listing.

Responsibilities

• Administers home health care to patients in their homes or in facilities as assigned.
• Develops and implements a written Plan of Care with physician orders and patient/family input.
• Performs nursing assessments, documents status and interventions accurately in charting.
• Delivers medications and treatments per agency policy and standards of care.
• Collaborates with the interdisciplinary team to adjust care plans as patient needs evolve.
• Educates patients and families on health plans, symptom management and end-of-life options.
• Participates in on-call rotation and makes visits as scheduled.
