Homewatch Caregivers of Shortpump/Midlothian is seeking a compassionate, organized RN to join our growing team on a PRN basis. The role focuses on in‑home, client‑centered care in Glen Allen, Shortpump, Midlothian and Chesterfield.

As Care Coordinator, you will conduct nursing assessments, develop individualized care plans, coordinate services, and build relationships with clients, families and healthcare professionals, while maintaining flexible scheduling and travel to client homes.

Qualifikationen

• Current, unrestricted RN license in Virginia, in good standing.
• At least one year in nursing, care coordination, case management, or home care preferred.
• Experience assessing clients and developing care plans preferred.
• Knowledge of HIPAA, confidentiality, and home care standards.
• Excellent communication, organizational, and interpersonal skills.
• Ability to work independently and manage assignments.
• Bachelor’s degree in Nursing preferred but not required.

As our Care Coordinator, you will play an important role in helping clients receive personalized, high-quality care in the comfort of their homes. You will conduct nursing assessments, develop individualized care plans, coordinate services, and build relationships with clients, families, and healthcare professionals.

Key Responsibilities

Client Assessments and Care Planning

• Conduct comprehensive nursing assessments to evaluate client needs, health conditions, preferences, and safety concerns.

• Serve as a primary point of contact for clients and families regarding care coordination.

• At least one year of nursing, care coordination, case management, or home care experience preferred.


• Experience conducting assessments and developing care plans preferred.


• Knowledge of HIPAA, client confidentiality, and home care standards.


• Excellent communication, organizational, and interpersonal skills.


• Ability to work independently and manage assignments effectively.


• Proficiency with electronic records management systems and Microsoft Office.


• Reliable transportation and willingness to travel throughout Henrico and Chesterfield counties.


• A bachelor's degree in Nursing or a related field is preferred but not required.
