Job summary

Personal-Touch Home Care seeks a Full-Time Registered Nurse for Start of Care (SOC) to plan, organize, and deliver home health services. You will assess patient needs, collaborate with physicians, and tailor care plans while involving patients and families in decisions.

Responsibilities include comprehensive assessments, IV administration, education, discharge planning, and supervising Home Health Aides. Requires VA RN licensure, CPR, driver’s license, and 1+ year of acute/long-term care

Qualifications

• Current licensure in the State of Virginia.
• CPR certification and a valid driver's license.
• Preferably, a minimum of one year of recent acute or long-term care experience.
• Bachelor's degree preferred, with at least one year of experience in home care.

Responsibilities

• Conduct comprehensive assessments of patients and families to determine home care needs.
• Provide thorough physical assessments and medical histories.
• Continuously reassess patient needs and adjust care plans accordingly.
• Collaborate with physicians to develop personalized care plans.
• Involve patients and families in the planning process.
• Implement preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed.
• Offer counseling and health education to patients and families.
• Identify discharge planning needs and implement plans prior to discharge.
• Perform peripheral short intravenous line establishment and administer IV medications under physician orders and with demonstrated competency.

Job Details Overview:
