Telos Health Systems, Inc. is seeking a licensed Physical Therapy Assistant (PTA) for a PRN position in Port Orange, Florida. You will deliver hands-on therapy under a licensed Physical Therapist, supporting resident care in senior living and home health settings.

Qualifications

• Associate degree from an accredited PTA program.
• Current Florida PTA license required.
• Strong communication and documentation skills.
• CPR certification as required by facility guidelines.

Responsibilities

• Implement physical therapy treatment plans as directed by a Physical Therapist.
• Monitor and document resident progress and response to treatment.
• Educate residents and caregivers on exercises and safe movement techniques.
• Communicate with interdisciplinary teams to ensure coordinated care.
• Maintain accurate clinical records in compliance with Florida guidelines.
