TELOS Health Systems in Port Orange, FL is seeking a licensed Outpatient Physical Therapist (PT) for a PRN role. You will assess, develop, and implement personalized treatment plans to help residents regain movement and reduce pain in senior living settings.

Qualifications

• Florida licensed Physical Therapist (PT) in good standing.
• Doctorate or Master’s degree in Physical Therapy.
• CPR certification as required by facility or state.

Responsabilités

• Perform evaluations and develop individualized treatment plans.
• Deliver skilled physical therapy services to residents in senior living settings.
• Monitor and document resident progress, adjusting plans as needed.
• Educate residents, caregivers, and staff on mobility, exercises, and prevention.
