Job summary

Avalon Village is seeking a Physical Therapist Assistant to provide skilled therapy under a registered PT supervision. This PRN role serves patients across ASC Therapies & Wellness Settings, including home health, SNF, and outpatient services.

The PTA will deliver direct therapy per the plan of care, monitor progress, support discharge planning, and collaborate with licensed therapists and staff to maintain compassionate, patient-centered care across our senior communities.

Qualifications

• Provide direct therapy to patients outlined in the OT's plan of care
• Monitor and document patient progress
• Collaborate with members for discharge plans
• Contribute to case management
• Maintain positive level of interaction with patients and center staff
• Adhere to applicable state regulations concerning occupational therapy
