Job summary

Accommodate, Restore & Collaborate, LLC seeks a PRN Physical Therapist Assistant (PTA) for Meadowbrook Nursing & Rehabilitation in LaGrange, IL. You will provide treatment based on plans created by the supervising therapist and document accurately.

The role requires Illinois PTA license, strong communication with patients and care team, and comfort with computers. SNF experience is preferred but other settings considered.

Qualifications

• Must hold an Illinois Physical Therapist Assistant license.

Responsibilities

• Provide treatment based on evaluations and plans of care by supervising therapist.
• Accurately complete patient documentation and maintain records.

• Communicate patient status and needs to patients, families, and caregivers.
• Collaborate with supervising therapist on patient progress.
• Engage in ongoing professional development through education.
• Provide in-service training to the interdisciplinary team as needed.
• Support department operations and meetings.
