Job summary

Accommodate, Restore & Collaborate, LLC in Bolingbrook, IL is seeking a PRN Physical Therapist Assistant (PTA) to support Meadowbrook Nursing & Rehabilitation. You will provide treatment based on plans of care created by the supervising therapist and help maintain quality resident care during assigned shifts.

Candidates must be licensed in Illinois as a PTA, able to communicate clearly with patients and families, and comfortable using computers.

Qualifications

• Licensed in Illinois as a Physical Therapist Assistant.
• Strong communication with patients, families, supervisors, and colleagues.
• Comfortable using computers.
• Experience in SNF preferred but not required.

Responsibilities

• Provide treatment based on evaluations/plans of care by supervising therapist.
• Complete documentation accurately and timely.

• Communicate patient status to patients, families, caregivers, and care team.
• Collaborate with supervising therapist on progress.
• Support department operations and meetings.

• Providing treatment based off evaluations/plans of care created by the supervising therapist.


• Appropriately and effectively completing documentation.


• Screening residents and individuals who may benefit from therapy.


• Communicating patient status and needs to the patients, family, caregivers, or other members involved with patient care.


• Collaborating and updating the supervising therapist regarding the progress of the patients.


• Expanding and enhancing self, in both clinical and professional knowledge through associations and continuing education.


• Providing in-service training sessions or other educational presentations to members of the interdisciplinary team as needed.


• Supporting and participating in department operations/meetings.

Requirements

• Licensed in the state of Illinois as a Physical Therapist Assistant.


• Ability to communicate well with patients, families, supervisors, and colleagues.


• Comfortable with computers.


• Preferred experience working in a SNF, but we are open to other practice area experience.
