Job summary

Accommodate, Restore & Collaborate, LLC seeks a PRN Physical Therapist Assistant (PTA) for Meadowbrook Nursing & Rehabilitation in LaGrange, IL. You will provide treatment based on plans created by the supervising therapist and document accurately.

The role requires Illinois PTA license, strong communication with patients and care team, and comfort with computers. SNF experience is preferred but other settings considered.

Qualifications

• Must hold an Illinois Physical Therapist Assistant license.
• Ability to communicate effectively with patients, families, and team members.
• Comfortable using computers and basic software for documentation.

Responsibilities

• Provide treatment based on evaluations and plans of care by supervising therapist.

• Collaborate with supervising therapist on patient progress.

Accommodate, Restore & Collaborate, LLC is seeking a PRN Physical Therapist Assistant (PTA) for Meadowbrook Nursing & Rehabilitation in LaGrange, IL. The job duties include, but are not limited to, the following:

• Providing treatment based off evaluations/plans of care created by the supervising therapist.
• Appropriately and effectively completing documentation.
• Screening residents and individuals who may benefit from therapy.
• Communicating patient status and needs to the patients, family, caregivers, or other members involved with patient care.
• Collaborating and updating the supervising therapist regarding the progress of the patients.
• Expanding and enhancing self, in both clinical and professional knowledge through associations and continuing education.
• Providing in-service training sessions or other educational presentations to members of the interdisciplinary team as needed.
• Supporting and participating in department operations/meetings.

Requirements

Requirements are as follows:

• Licensed in the state of Illinois as a Physical Therapist Assistant.
• Ability to communicate well with patients, families, supervisors, and colleagues.
• Comfortable with computers.
• Preferred experience working in a SNF, but we are open to other practice area experience.
