Job summary

Accommodate, Restore & Collaborate, LLC is seeking a full-time Physical Therapist at Meadowbrook Nursing & Rehabilitation in LaGrange, IL. The PT will assess, diagnose, and treat gait, mobility, and functional impairments in adult patients at this site, performing evaluations and documenting plans of care.

Responsibilities include patient communication, supervising assistants as applicable, continuing education, and participating in department operations.

Qualifications

Accommodate, Restore & Collaborate, LLC is seeking a full-time Physical Therapist at Meadowbrook Nursing & Rehabilitation in LaGrange, IL. We are looking for a PT to prevent, assess, diagnose, and treat gait, mobility, and function in adult patients at this site.

The job duties include, but are not limited to, the following:

• Performing evaluations/plans of care and providing treatment accordingly.
• Appropriately and effectively completing documentation.
• Screening residents and individuals who may benefit from therapy.
• Communicating patient status and needs to the patients, family, caregivers, or other members involved with patient care.
• Directing and supervising the work of assistants as applicable.
• Expanding and enhancing self, in both clinical and professional knowledge through associations and continuing education.
• Providing in-service training sessions or other educational presentations to members of the interdisciplinary team as needed.
• Supporting and participating in department operations/meetings.

Requirements

Requirements are as follows:

• Licensed in the state of Illinois as a Physical Therapist.
• Ability to communicate well with patients, families, supervisors, and colleagues.
• Comfortable with computers.
• Preferred experience working in a SNF, but we are open to other practice area experience
