Job summary

The Heritage Center and Horizon's rehabilitation is seeking a Licensed PRN Physical Therapy Assistant to join our team. In this role, you'll provide PTA services tailored to each patient's treatment plan in line with our mission to deliver compassionate, high-quality care.

You will deliver therapy services per plans, implement procedures, and maintain accurate records while complying with agency policies and care standards.

Qualifications

• Graduate of accredited Physical Therapy Assistant Program.
• State credentialing/licensure as required.
• Experience in long term care and membership in professional organization is preferred.

Responsibilities

• Deliver individualized therapy services as prescribed in treatment plans.
