Job summary

We are seeking a compassionate and skilled Physical Therapist (PT) to join our in-house rehabilitation team at Newport Meadows Nursing and Rehabilitation Center in Christiana, Pennsylvania. About the Role:

As a PRN PT, you will be responsible for providing physical therapy services to our patients, including assessment, treatment, and education. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient. You will also be responsible for maintaining accurate records, updating patient files, and communicating with physicians and other healthcare professionals.

Responsibilities:

• Conduct thorough patient assessments to identify physical therapy needs and develop individualized care plans
• Implement and conduct physical therapy treatments, including modalities, exercises, and other interventions
• Monitor and document patient progress, adjusting treatment plans as needed
• Collaborate with the multidisciplinary team, including physicians, occupational therapists, speech therapists, and other healthcare professionals
• Maintain accurate and up-to-date patient records, including progress notes and treatment plans
• Participate in patient care conferences, discharge planning, and other interdisciplinary meetings
