Job summary

Care Pavilion Nursing & Rehabilitation Center in Philadelphia, PA seeks a Full-Time Physical Therapist to join our in-house rehab team. We offer PRN opportunities and a supportive environment focused on mobility and quality of life for residents.

As part of the rehab department, you’ll collaborate with nursing and facility leadership, develop individualized plans, and document progress to ensure positive resident outcomes in a skilled nursing setting.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Pennsylvania Physical Therapist license or eligibility for licensure.
• Strong communication and clinical skills.
• Ability to work effectively as part of an interdisciplinary team.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy services to SNF/LTC residents.
• Help residents improve strength, mobility, balance, safety, and functional independence.
• Establish measurable functional goals and monitor resident progress.
• Complete accurate and timely documentation.
• Collaborate with interdisciplinary team to support resident outcomes.
• Provide resident, family, and caregiver education as appropriate.

Care Pavilion Nursing & Rehabilitation Center in Philadelphia, PA is seeking a Full-Time Physical Therapist (PT) to join our in-house rehabilitation team. We also have PRN opportunities available for therapists looking for additional flexibility.
