Job summary

CareMed Health Solutions seeks a Nurse Practitioner to provide primary and preventative care for residents in skilled nursing facilities. You will manage acute changes in condition, perform advanced care planning, and prescribe medications in accordance with Kentucky law.

This role requires Board Certified NP with Kentucky licensure, DEA credentials, and experience in geriatric or adult care within a long-term care setting.

Qualifications

• Current, unrestricted NP license in Kentucky.
• Board Certification.
• DEA licensure.

Responsibilities

• Provide primary and preventative care for long term care patients in skilled nursing facilities.
• Manage residents with acute changes in condition.
• Advanced care planning and chronic care management.
• Prescribe medications, including controlled substances per KY law.
• Order, review and follow up on treatments and diagnostic testing.
• Communicate with nursing home staff regarding changes in condition and care plans.
• Collaborate with primary care physician.

NP ROLE AND RESPONSIBILITIES:

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with KY Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with primary care physician.

Np Qualifications And Education Requirements

• Current, unrestricted NP license in the state of Kentucky.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.
