Job summary

CareMed Health Solutions in Bowling Green is seeking a Nurse Practitioner to provide primary and preventative care to residents in long-term care facilities. You will work as part of an interdisciplinary team to manage chronic conditions and coordinate care with facility staff.

The role includes advanced care planning, prescribing medications in accordance with KY law, and collaborating with a primary care physician. Experience in geriatrics or family practice and EMR proficiency are preferred.

Qualifications

• Current, unrestricted NP license in Kentucky.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team.
• Basic coding knowledge preferred, not required.

Responsibilities

We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES:

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with KY Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with primary care physician.

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS
