Job summary

CareMed Health Solutions seeks a Nurse Practitioner to deliver primary and preventive care to residents in skilled nursing facilities in Kentucky. You will manage acute changes, engage in advanced care planning, and prescribe medications in accordance with Kentucky law.

The role requires board certification, KY licensure, DEA, and experience in geriatric care within an interdisciplinary team. EMR proficiency and collaboration with PCPs are essential for success.

Qualifications

• Current, unrestricted NP license in Kentucky.
• Board Certification as NP.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to work within an interdisciplinary care team.

Responsibilities

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with KY Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition and the care plan.
• Collaborate with primary care physician.
