Job summary

Santa Rosa Post Acute in Santa Rosa, California, is seeking a part-time Staff Physical Therapist Assistant (PTA) for PRN coverage. The PTA assists the Physical Therapist with resident care, documents promptly, and supports safety and infection control protocols.

You will participate in care conferences, educate residents and families, and help with discharge planning while coordinating with the supervising PT and interdisciplinary team. A California PTA license and Associate Degree are required.

Qualifications

• Associate Degree from an accredited PTA program.
• Proficient in computer skills such as inputting data into Net Health and Point Click Care.
• Licensed as a Physical Therapist Assistant in the state.
• CPR certification, if required by licensure.

Responsibilities

• Treat residents as directed by Physical Therapist.
• Complete timely, accurate, and compliant documentation.
• Maintain a safe treatment environment per protocols.
• Participate in Resident Care conferences and Rehabilitation meetings.
• Assist nursing with training of Restorative Aides.
• Communicate with supervising PT on progress and discharge planning.
• Educate residents and families on treatment goals and home exercise programs.
• Assist with ordering/facilitating necessary durable medical equipment.
