Position: PRN Physical Therapy Assistant

Edenbrook Lakeside, a skilled nursing facility within the Eden Senior Care Network, is seeking a PRN Physical Therapy Assistant to provide rehabilitation services across our locations. Eden Senior Care, founded in 2016, is a growing healthcare management company focused on managing Skilled Nursing, Rehabilitation, and Assisted Living communities in Minnesota, Wisconsin, and Ohio. Job Responsibilities:

1. Maintain rehabilitation service standards aligned with the company's mission and goals.
2. Provide courteous and respectful care to all patients, respecting their environment and culture.
3. Deliver quality therapy services, including treatment interventions, program implementation, discharge planning, documentation, and communication, under the supervision of a physical therapist.
4. Manage a work schedule appropriate for the facility and in line with employment terms.
5. Support positive relationships with facility staff and adhere to operational policies and procedures.
6. Screen patients for evaluation needs under PT supervision.
7. Treat patients to relieve pain and restore or develop function to achieve maximum performance, under PT supervision.
8. Monitor patient responses, communicate changes, and update treatment plans accordingly.
9. Complete documentation accurately and promptly.
10. Collaborate with team members regarding patient care.
11. Assist in collecting data on patient progress under PT supervision.
12. Coordinate discharge planning when maximum benefits are achieved.
13. Provide accurate billing summaries.
14. Participate in in-service education and maintain an organized therapy department.
15. Perform administrative duties related to therapy services.
16. Identify new rehab admissions and communicate with PT staff.
17. Notify staff of upcoming discharges and attend care plan meetings as requested.
18. Provide in-service education to staff when needed.
19. Supervise physical therapist assistant students appropriately.
20. Perform other related duties as assigned.
