Job summary

Creative Solutions In HealthCare is seeking a compassionate Physical Therapy Assistant (PTA) to join our in‑house rehab team on a PRN basis. You will administer prescribed PT treatments and work under a Physical Therapist to support mobility, strength, and quality of life for residents.

We value strong communication and teamwork, and welcome new graduates. The role offers flexible scheduling and in-house rehab support within a SNF/long-term care setting.

Qualifications

• Active and valid Texas Physical Therapy Assistant license
• In good standing with all regulatory agencies and licensing boards
• SNF or long-term care experience preferred
• Current knowledge of physical therapy treatment practices
• Experience with Net Health Electronic Documentation System preferred
• Ability to manage patients with varying personalities and needs
• Strong communication and teamwork skills

Responsibilities

• Administer medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing
• Provide skilled physical therapy services and interventions under the supervision of a Physical Therapist
• Respond to requests for service by relaying information and referrals to the Physical Therapist
• Implement and modify patients’ plans of care under PT supervision
• Measure and record patients’ motor function, strength, and muscle performance
• Monitor patient response to treatment and adjust approaches with the team
• Provide clinical support and education to patients, families, and caregivers
• Ensure documentation complies with policies and regulatory requirements
• Attend required meetings with the Director of Rehab
