Job summary

Enhance Rehabilitation in Milwaukee, WI is seeking a qualified Physical Therapist to join our SNF setting. You will evaluate and treat patients following an individualized plan of care, while coordinating with facility staff to ensure high-quality care and patient outcomes.

You will supervise Physical Therapist Assistants, re-evaluate progress, adjust plans of care, and maintain thorough documentation of evaluations, progress notes and discharges to support patient recovery.

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including evaluations, re-evaluations discharges and daily progress notes
