Evolve Therapy Services is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility in Omaha, NE. The PRN role focuses on planning, directing, and delivering physical therapy in alignment with federal, state, and local standards to ensure high-quality resident care.

Responsibilities include screening residents, developing treatment plans, and communicating progress to residents, families, and the interdisciplinary team.

Qualifications

• Timely and compliant documentation aligned with CMS expectations.
• Collaborate with interdisciplinary team to plan and implement therapy.
• Perform resident screening and develop short/long-term goals.

Responsibilities

• Provide PT services to residents per standards and procedures.
• Adhere to documentation and billing procedures.
• Evaluate residents and develop treatment plans with goals.
• Communicate findings and plans to residents, families, and team.
• Complete required CMS documentation in NetHealth EMR.
