Job summary

Life Care Centers of America is seeking a Physical Therapist to provide direct rehab services to patients referred by physicians. You will implement treatment plans, set functional goals, and collaborate with an interdisciplinary team to improve patient outcomes in a SNF environment.

Ideal candidates hold Idaho PT licensure, have completed an accredited PT program, and maintain CPR certification. Experience preferred, with new grads welcome; role is PRN, based on facility needs.

Qualifications

• Graduation from an accredited physical therapy program (BSPT/MSPT/DPT); MSPT/DPT preferred.
• Currently licensed/registered in Idaho and maintain active license.
• CPR certification required upon hire or during orientation.

Responsibilities

• Follow established physical therapy treatment plans for patients.
• Establish and modify functional goals with measurable outcomes.
• Supervise PTAs, Rehab Aides, and PTA students; oversee care delivery.
• Document patient progress accurately and timely; use therapy software.
